If you are already a patient and moving locally
If you are a current registered patient of Mayfield Medical Centre and you move from our catchment area but still live within the area bordered by the A325 / B3014 and Minley Road you can remain registered with us but if in doubt, please do check with us. You can also view the catchment map in Reception.
You always have a choice to register elsewhere if the area you live in is also within another surgery’s catchment area.
If you move out of Mayfield Medical Centre’s catchment areas you will need to re-register with another GP Surgery.

To register as a patient
Please complete the relevant form(s) below, which are also available from reception.
When you first join the Practice you are invited to share medication information or any illness aspects which you are currently being treated for on the application form and this may result in a request to make an appointment with a doctor or practice nurse to discuss your on-going health care.
All patients are assigned to a named GP on registration who will then normally be your usual GP although whilst we strive to do so, we can not always guarantee that you will always see your usual GP when you wish to make an appointment.
Please note that when you register your notes may be sent electronically from your previous doctor and likewise when you leave the Surgery they may be sent electronically to your new doctor. Your personal information may be shared within the healthcare team and with other health providers. If you have objections to either of the above, please write to the Practice Operational Manager.
Please download and complete the GMS1 Family doctor services registration form for each member of your household who needs registering, plus EITHER an Adult OR Child registration checks and short questionnaire for for each household member.
