Patients who wish to register with the practice must live within the catchment area displayed in the reception lobby. Please complete a registration form which is available from reception. When you first join the Practice you are invited to share medication information or any illness aspects which you are currently being treated for on the application form and this may result in a request to make an appointment with a doctor or practice nurse to discuss your on-going health care.



All patients are assigned to a named GP on registration who will then normally be your usual GP although whilst we strive to do so, we can not always guarantee that you will always see your usual GP when you wish to make an appointment.



Please note that when you register your notes may be sent electronically from your previous doctor and likewise when you leave the Surgery they may be sent electronically to your new doctor.  Your personal information may be shared within the healthcare team and with other health providers.   If you have objections to either of the above, please write to the Practice Manager.



The Practice operates a zero tolerance policy on abuse or violence and in the event of violent or abusive behaviour by a patient to doctors, nurses, any member of staff or persons present on the practice premises, we have the right to remove the patient from the premises, if necessary with the help of the Police, and request their immediate removal from the practice list.  Equally you will be given the respect and courtesy expected from all our staff at the surgery.
