Patient Extended Access

 

As part of locality working, the seven Farnborough practices are working together to share the provision of GP Extended Access.  These practices are Alexander House, Giffard Drive Surgery, Jenner House Surgery, Mayfield Medical Centre, Milestone Surgery, North Camp Surgery & Southwood Practice,

 

 

The Farnborough locality practices have been working on establishing the best way to offer our patients GP Extended Access, taking into consideration the available resources.

 

The best solution is to share this workload by offering a rotating system of evening and Saturday morning clinics with either a doctor (GP) or a nurse.   Each of the seven participating practices could possibly see a combination of their own patients and patients from the other practices listed above. 

 

You will have the opportunity to be seen at Mayfield Medical Centre on a Wednesday and/or Saturday mornings/Friday evenings on a rota basis outside of the NHS contracted Core Hours (between 8.30am and 6.30pm Monday to Friday) or at another of the participating practices dependent on the duty rota.  Your own practice will be able to advise on this.

 

To enable a GP/nurse to see you at a practice that is not the one you are registered with, you will need to agree (consent) for that GP/nurse to view your medical records as part of your care.  Without your verbal consent they do not have access to all your information and the appointment cannot be given.  This access to the patient’s medical record is restricted to the GP/nurse with whom you have booked your appointment, for the appointment itself, and for 28 days afterwards.  This period of 28 days following the appointment is to allow the clinician to complete necessary paperwork and follow up where necessary. 

 

All of the practices in the North East Hants and Farnham CCG Locality use EMIS Web Clinical System.

 

As with any new system there will be a period of adjustment and teething to work through.  We thank all our patients in advance for their consideration during this time and would ask that you consider the alternative methods for access to care if an appointment is not immediately available for you, or at a time that suits:

 

  • GP Telephone Triage/Consultation
  • Local Pharmacy advice

 

 

UPDATE

By March 2019 everyone in England will benefit from access to general practice appointments in the evenings and weekends at a time that is most convenient to them. This is part of a national drive to help improve access to general practice and get the best possible outcomes for patients. Further information is available at www.england.nhs.uk/gpaccess
