Important GDPR Notice




From 3 September 2018, in order to comply with the General Data Protection Regulation that came into effect on the 25 May 2018, you will be asked to show photo identification when collecting a prescription or any paperwork from the practice.   


You can bring a passport, driving licence or bank card.


If you ask someone to collect your prescription or paperwork on your behalf, we will need prior notice of their name & relationship to you so a note can be added to your medical record.


Whoever collects your prescription will need to bring their own photo identification.






GDPR and the National Data Opt Out

On 25th May, the Data Protection Act 2018 comes into force. Mayfield Medical Centre has ensured that we have met our responsibilities under the new legislation. Full details can be found under the "Your Medical Record" tab on the left menu option.


In particular, you can find:

  • Our Privacy policy
  • Detailed privacy notices for all data processing that we perform on our patients' medical information
  • Your data subject rights
